Welcome to the School Year!
If your child needs medication at school, please see the Medication Permission Form for our detailed school policy. School policy does not allow students to carry medication. If a student is required to take medication during school hours, a medication permit form is required. The state is available in the school office or from this site on the left side under documents. Papers must be signed and turned in with any medication.
Please let me know if your child has any changes in their medical history or alleriges. Any student with allergies requiring an epi-pen, asthma, seizures disorder or diabetes will need an action plan signed and dated by the physician. It is required that the school be provided with any prescribed emergency medications to be administered if indicated. Additionally, a medication permission form will need to be completed and signed by a parent or guardian. We have sample allergy, asthma, and diabetes action plans on the left side under documents. When your child has required immunization, please send or drop off a copy in the school office.
Annual physicals are required for all student-athletes. Athletic competition Health Forms are available in the school office or from this site.
The school nurse's hours are Monday-Friday, 9:00 am- 3:00 pm.

illness and should not attend school, regardless of whether the sickness is COVID-19. For anyone with
